Individual Staff Risk Assessment for Covid-19 – version 7, September 2021

All staff need to complete a Covid risk assessment using our new Digital Risk Assessment tool Version 7 – even if a previous hard copy version of a risk assessment has been done.

During the Covid-19 pandemic we have evolved our Staff Individual Covid-19 Risk Assessment to be as comprehensive and applicable as possible using the latest evidence and guidance.

We have now entered a new world. To respond to this, we have incorporated the latest evidence by using the Covid age tool. This evidence-based tool enables better assessment of multiple risk factors. In addition to the Covid age tool, shielding advice, pregnancy and Covid vaccination history are also incorporated in assessing individual risk. This results in an even more sophisticated outcome tool.

The tool is aimed at identifying staff vulnerability and if they are very high risk (clinically extremely vulnerable (CEV)), high risk, increased risk or no increased risk.

It is expected that all staff complete this digital risk assessment at least annually whilst Covid remains a risk, or sooner than that if there have been any significant changes to their health that may affect their risk group.

All CUH staff (substantive, fixed term, temporary, bank, volunteers)

  • Log in with your normal CUH log in details
  • Read the covering note
  • Click the consent box
  • Continue to complete your form
  • Explanatory information is included throughout to help you
  • You need to complete every section in order to submit your Risk Assessment

For Medirest and other staff (those who do not have a CUH log in)

Once the risk assessment is complete
Once the risk assessment is complete you and your manager will be emailed the outcome:

  • You will receive the details of your submission and a copy of this is sent to Occupational Health and Wellbeing.
  • The email to your line manager will include:
    • Your risk group
    • Your self-reported vaccination status
    • Whether you have indicated that you would like a further management discussion
    • Whether you have declared that you live with someone who is Clinically Extremely Vulnerable (CEV).
  • The manager’s email will not include other medical or personal information. By completing this digital Covid-19 Risk Assessment you are consenting to your line manager being sent this information. Having this information will enable your line manager to ensure the necessary workplace adjustments are in place.
